What to Expect
When you arrive for your first visit, you will be greeted and asked to complete paperwork. A medical assistant will bring you into an exam room to get your vitals, review medications , medical history and establish why you are being seen. Your provider will be in to discuss the reason for your visit and will partner with you to create an individualized plan of care that best suits your unique needs. We ask new patients to arrive 30 minutes prior to their scheduled appointment time. Please be proactive and have relevant medical notes, imaging and labs sent to our office from other providers prior to your appointment. This will be helpful in establishing a treatment plan, without having to repeat testing.

What to Bring to Your Appointment

  • Government-issued Photo ID.
  • List of all medications you are currently taking (names and doses). Many patients find it easier to simply bring their medication bottles with them.
  • Insurance card(s).
  • A parent or legal guardian if the patient is under 18. A parent or legal guardian must be present during the entire visit.
  • Records from other health care providers, if applicable.
  • Completed forms including New Patient Questionnaire and Registration Form (see below for forms).
